State Violence Prevention Grant Reporting Information
Grantees will be required to submit one Progress and Expense Report via DocuSign. An example of the reporting form for State Violence Prevention Grantees is provided below for reference. Additionally, performance measures will be collected online to show the impact of the State Violence Prevention Grant Program and a copy has been provided below for reference.
