Customer Setup Form (DOA-6453)	Create a New/Modify Existing Customer
Customer Contact Information (DOA-6454)	Create a New/Modify Existing Customer Contact
STAR Sponsor Information (DOA-6455)	Create a New/Modify Existing Sponsor
When you have completed the appropriate forms, you must LOCK them and submit to the STAR Customer Support page.
