Application Materials

Instructions: The COVID-19 Pandemic Response Nonprofit Grant Program application requires two steps, both of which MUST be completed prior to the November 9, 2020 at 2:00 pm CT application deadline.

  1. Complete the Grant Application survey
  2. Complete both Attachment A: Attestation and Attachment B: IRS Form W-9 and submit to DOACovidNonprofitGrant@wisconsin.gov



Timeline
  • Eligible Expense Incurred Period - March 1, 2020 through December 30, 2020
  • Grant Announcement Issue Date - Monday, November 2, 2020
  • Webinar - Wednesday, November 4, 2020 at 1:00 PM CT (see "Resources" for instructions)
  • Applications Due - Monday, November 9, 2020 at 2:00 PM CDT
  • Notification of Award Issue Date - November 2020 (estimate)
  • Grant Agreement Execution - November 2020 (estimate)
  • Final Report Due - February 1, 2021
